DESCRIPTION:
The District Two Adult Recovery Court Case Manager serves as
a key member of the multidisciplinary treatment court team and supports
participants involved in the Adult Recovery Court program. This position is
responsible for providing comprehensive case management services that promote
participant recovery, accountability, treatment engagement, and successful
program completion
The Case Manager will conduct initial and ongoing
assessments of participant needs, develop individualized case plans, coordinate
referrals to treatment and recovery support services, monitor participant
progress, and maintain regular contact with participants throughout their
involvement in the program. The position requires a compassionate,
participant-centered approach while maintaining adherence to evidence-based
treatment court practices.
REQUIREMENTS:
Minimum of High School Diploma. Experience in case
management, human services, behavioral health, criminal justice, social
services, or related field.
