Martin-Tyrrell-Washington District Health Department is seeking a qualified Social Worker to provide case management services for CMHRP and CMARC for Martin and Washington Counties.
The CMHRP Care Manager is a health professional who assists pregnant clients in accessing the health care and social services systems in order that outcomes which contribute to physical and emotional health and wellness can be obtained. The major goals of the CMHRP program are to reduce infant mortality through improved pregnancy outcomes; ensure the provision of comprehensive preventive health services to pregnant women and their infants; and assist pregnant women to meet other priority needs that affect their well-being and that of their families. Care management services are offered through outreach, conducting assessments, service provision, coordination, referral, follow-up/ monitoring and advocacy. Care Managers work closely with the patient, her support system, medical providers as well as other community service providers. Contact is made with the patient by home visits, community encounters as well as at the pregnancy medical home practice visits.
The CMARC Care Manager is a health professional who assists families with children birth to 5 years old in accessing the health care and social services systems in order that outcomes which contribute to physical and emotional health and wellness can be obtained. The work involves the responsibility for providing professional social work services for the CMARC program with Martin-Tyrrell-Washington District Health Department. An employee of this class is primarily responsible for providing assessment, intervention, client education and high level comprehensive and intensive care management services to eligible high-risk children with complicated personal, psycho-social, medical, developmental and economic needs.
Minimum Education and Experience:
Master’s in social work from an appropriately accredited institution and one year of directly related experience; bachelor’s degree in social work from an appropriately accredited institution and two years of directly related experience.
Necessary Special Qualifications:
Master’s or bachelor’s degree in social work.
